By proceeding with this booking, the Client acknowledges and agrees to the following terms:

1. Service & Deposit: The Client is booking a deposit for the "Upper Face Botox" treatment area at Mysa Med Spa. This payment is a deposit securing the appointment and is applied toward the final treatment cost, which will be determined during the in-person consultation.

2. Consultation-Based Treatment: The Client understands that the final treatment plan, including the exact number of units required, will be determined during a mandatory consultation with a Mysa Med Spa Nurse Practitioner immediately prior to the procedure. The flat-rate price covers the treatment area as determined by the medical professional.

3. Neurotoxin Selection: The Client will make their final selection of neurotoxin brand (Allegan Botox®, Daxxify®, or Letybo®) during the in-person appointment, after consultation with the treating practitioner.

4. Pre-Treatment Requirements: The Client acknowledges they have been informed of and agree to adhere to pre-treatment care instructions. This includes avoiding alcohol, blood-thinning medications (e.g., aspirin, ibuprofen), and topical products containing Retinol or Vitamin A for at least 24 hours prior to their appointment to minimize the risk of bruising and swelling.

5. Allegan Rewards Points Policy: The Client understands and agrees that Mysa Med Spa's "Botox by Area" pricing is a flat-rate promotional model. Therefore, Allegan Rewards Points cannot be applied or redeemed against treatments purchased under this specific pricing structure. Points are only eligible for redemption on treatments purchased on a traditional per-unit basis.

By completing this booking, you certify that you have read, understood, and voluntarily agree to these terms.

• Avoid drinking alcohol for at least 24 hours prior to your appointment.

• Stop taking any blood thinning medications like Aspirin for 24 hours.

• If you are on any prescription medication or prescription blood thinners like Warfarin, please call us and let us know prior to booking an appointment.

• If possible, do not wear makeup to your appointment.

This depends on the procedure you get and your body’s unique response to the procedure. However, we advise everyone to expect their final results within 10-14 days after filler procedures and 4-7 days after botox procedures.

You will receive a Mysa “healing Packet” with more information about your procedure and what to expect healing-wise after your procedure.

Your after-care instructions packet will be handed to you after your procedure. However, here are some things you should know:

• No Intense workouts for the first 24 hours

• No makeup for the first 24 hours

• No Retinol products or creams for the first 24 hours

• No saunas, steam rooms, hot tubs or other excessively hot environments for the first 24 hours

• Our cancellation policy requires that appointments be cancelled at least 48 hours in advance to avoid any extra charges. However, all deposits are non-refundable but transferable.

• If you cancel within 24 hours of your appointment, it can be rescheduled, but a second deposit will be necessary.

• In case of a no call/no show, you have up to 24 hours to contact us to avoid being blacklisted.

• If you are more than 20 minutes late, your appointment will need to be rescheduled with a second deposit.
